Overview
Lung Cancer Screening (LCS) is a module in Rad AI Continuity that’s dedicated to Lung‑RADS screening and follow-up, with worklists, a longitudinal timeline, and EHR‑driven automation to close the loop on Low‑Dose Computed Tomography (LDCT) follow‑ups.
What you can do
- Review and monitor patient inclusion in the Lung Cancer Screening program by reviewing eligibility status, confirming enrollment status, and confirming the detected Lung-RADS category.
- Track patient progress with prioritized worklists, due dates, and automated HL7‑based status updates.
- Submit records to the ACR’s Lung Cancer Screening Registry (LCSR) and track their progress.
- Monitor your LCS program’s performance with analytics and saved views.

Switching to the Lung Cancer Screening Module
Rad AI Continuity has two modules:
- Standard Module, covering a broad range of categories and subcategories including AAA, Fleischner, Thyroid, TI-RADS, and more.
- Lung Cancer Screening Module, dedicated to tracking the Lung-RADS category only—with additional workflows, like lung cancer screening program eligibility.
The current module is displayed in the top-right corner of your screen, next to your name. To switch to the Lung Cancer Screening Module:
- Click Standard.
- Select Lung Cancer Screening from the drop-down menu that appears. The worklists will update accordingly.
Navigator workflow
Here’s the recommended workflow for navigators working on Lung Cancer Screening:
- Select Entries from the main navigation menu at the top. Go through entries in the Pending review worklist, open them in the Patient View, and complete the following tasks:

Monitor the following worklists:
- Order approval (due): Approved entries that are waiting for the provider to place an order for the follow-up exam.
- Exam scheduling (due): Entries for which an exam has been ordered, but not yet scheduled by the patient.
- Exam completion (due): Entries for which an exam has been ordered and scheduled, but the patient has not yet completed the exam.
💡 Once the exam is completed, Rad AI will automatically close the loop and—if there’s a new Lung-RADS recommendation—move the patient back to the “Pending review” worklist under a new entry. - Go to the Registry and periodically complete, submit, and review records for submission to the American College of Radiology (ACR). Entries
The Entries area is where Continuity organizes all entries into focused worklists—each representing a different stage of the follow-up workflow. You can view, filter, and navigate these different worklists to manage patient follow-up efficiently.

Navigating default Lung Cancer Screening worklists
Default worklists are displayed on the left-hand side of the screen. Click one to navigate to that worklist.
Entries progress through these worklists, based on automated or manual status updates:

Main worklist: shows all entries in the system, except for those in “resolved” status (i.e., unenrolled from the LCS program).

Pending review: shows only entries in “pending review” status that require navigator review for eligibility, enrollment, and Lung-RADS categorization.
💡 This is the default worklist you’ll land on when you navigate to the “Worklists” page.
By default, the “Pending review” worklist displays the Lung-RADS classification under the Subcategory column, based on the contents of the corresponding report. -
Order approval (Due): shows only entries in “pending order approval” status awaiting the provider’s order for a follow-up exam.

Exam scheduling (Due): shows only entries in “pending exam scheduling” status. The order has been placed, and the patient is now responsible for scheduling the exam.

Exam completion (Due): shows only entries in “pending exam completion” status. The exam has been scheduled, but not yet completed. Once the exam is completed, Rad AI will automatically close the loop and—if there’s a new Lung-RADS recommendation—move the patient back to the “Pending review” worklist under a new entry.

Snoozed: shows entries that are temporarily hidden from other worklists until a specified date. Learn more about snoozing entries.
- Unenrolled: shows only entries in “resolved” status due to the patient being unenrolled from the LCS program. Learn how to unenroll a patient from the LCS program.
Understanding Lung Cancer Screening prioritization
By default, worklist entries are sorted by priority—from highest to lowest.
Priority is determined by the following factors:
- The clinical urgency of the Lung-RADS finding subcategory:

Patient View
Navigating entries
Select an entry from a worklist to open the Patient View.
Once in the view, you can:
- Click Return to {worklist} to go back to the worklist view
- Click Expand/Collapse to make information cards bigger or smaller
- Use the Previous/Next arrows to move through entries without having to return to the worklist
Viewing information

Patient Information: Displays key information about the patient—including name, MRN, date of birth, address, phone number, plus eligibility and enrolment status.
- Click the copy icon to quickly copy the patient’s MRN.
- Click the copy icon to quickly copy the patient’s MRN.

Notes: Displays notes that document patient preferences or important contextual information.
💡 To document communication with patients or providers, we recommend using the log contact action instead.- Click the + button to add a new note.
- To delete a note, select the note then click Delete Note.
- Click the + button to add a new note.

Exams: Lists prior imaging studies.
- Click the Show only LCS exams toggle to show or hide non-Lung Cancer Screening exams for that patient.
- Click an item to view a report on that exam.

History: Logs key events and provides an audit trail for this entry. For example, if the status of the entry changed from “pending review” to “order approval (due)”. Also used for logging communication with patients and providers. Learn how to log contact via the actions menu.
- Click an event to reveal more info, such as the user who carried out the activity.

Lung-RADS timeline: displays entries that represent the patient’s completed Lungs-RADS exams over time:
- Click Monthly to change the scope of the timeline:
- Daily
- Monthly (default)
- Annual
- Click the → and ← arrows—or click, hold, and drag—to scroll forwards and back through the timeline.
- Click the target icon to jump to the active entry.
- Click an entry to select it as the active entry. The section below will update to show the exam report for that entry. See “Report” below for more information.
- Click the three-dot icon on a timeline entry to reveal entry actions.
- Click Monthly to change the scope of the timeline:

Report (Accession): displays the exam report for the active entry, including findings and recommendations.
- Click the copy icon next to Accession to quickly copy the accession number.
- Click the copy icon next to Accession to quickly copy the accession number.

To Do: displays required steps for reviewing patient eligibility, confirming patient enrollment, and confirming the detected Lung-RADS category.
- The badged number indicates the number of remaining incomplete tasks.
- Completed tasks will appear checked.
- Click an item in the list to begin the task.
- Provider Information: Displays referring and following provider details.
Approving a pending review
You must complete three tasks before you can progress a patient to the next stage of the LCS program:
- Review the patient’s eligibility status
- Confirm the patient’s enrollment status
- Confirming the detected Lung-RADS category
Once these tasks are complete, click Approve and continue in the To Do section of the Patient View. The entry will move from the “Pending review” worklist to the “Order approval (Due)” worklist.
Reviewing a patient’s eligibility status
This workflow reviews a patient’s age, smoking pack-years, symptoms, and smoking status to confirm (or deny) their eligibility for enrollment in the LCS program.
- Select an entry from the Pending review worklist to open it in the Patient View.
- Under To Do, select Review patient eligibility status to open the Patient Eligibility dialog.

Click on the fields and enter/select the patient’s Smoking pack-years, Symptoms, and Smoking Status
💡 The patient’s Age is populated automatically.- (Optional) On entering the patient’s smoking status, you can also set the date they quit smoking by clicking Set quit date and selecting a date from the calendar.
- (Optional) On entering the patient’s smoking status, you can also set the date they quit smoking by clicking Set quit date and selecting a date from the calendar.

(Optional) Click Add note, enter a note, then click Save.
💡 To view a note that’s been added, click View note. From here, you can Edit the note or delete it by clicking the trash icon. - Click Update eligibility. The patient’s eligibility status will be updated, depending on whether they meet the eligibility requirements or not:
- (Requirements met) Eligibility status updates from “Potentially eligible” to Eligible”
- (Requirements not met) Eligibility status updates from “Potentially eligible” to “Ineligible”
Eligibility requirements
By default, all patients in the Lung Cancer Screening module are automatically enrolled in the LCS program. However, navigators must still review patient eligibility for compliance purposes.
To confirm a patient’s enrollment in the LCS program, they must meet the following requirements according to U.S. Preventive Services Task Force (USPSTF) guidelines:
- Age: between 50 and 80 years old
- Smoking pack-years: at least 20
- Symptoms: asymptomatic
- Smoking status: current smoker, or quit within the last 15 years
Confirming a patient’s enrollment status
By default, all patients in the Lung Cancer Screening module are automatically enrolled in the LCS program. However, navigators must still confirm patient enrollment for compliance purposes.
- Select an entry from the Pending review worklist to open it in the Patient View.
- Under To Do, select Confirm patient’s enrollment status to open the Patient Enrollment dialog.
- Review the patient’s Enrollment Status. By default, all patients in the Lung Cancer Screening module are automatically marked as “enrolled”. However, you may wish to unenroll a patient from the LCS program.
- (Optional) If Shared Decision Making (SDM) and smoking cessation counseling has taken place, check the box: Shared decision-making (SDM) and smoking cessation counseling has occured.

Click Confirm status.
⚠️ This action is only available once you’re reviewed and updated the patient’s eligibility status, and confirmed they meet the eligibility requirements. You can also confirm enrollment for an ineligible patient with an override.
The action also becomes available if you change the patient’s enrollment status to “unenrolled” and select a reason for unenrollment. Learn how to unenroll a patient from the LCS program.
Overriding enrollment for an ineligible patient
If a patient doesn’t meet the USPSTF eligibility requirements after you’ve reviewed their eligibility status, you won’t be able to confirm their enrollment status unless you perform an override and provide a reason.
- Select an entry for an ineligible patient from the Pending review worklist to open it in the Patient View.
- Under To Do, select Confirm patient’s enrollment status to open the Patient Enrollment dialog.
- Click Reason for override and select a reason from the drop-down menu:
- Secondhand smoke exposure
- Family history of lung cancer
- Occupational exposures
- Lung disease history
- Prior indeterminate or suspicious nodules
- Shared decision-making preference
- Other
- (If “Other” selected as reason for override) Click Please specify reason and enter the reason why you’re overriding enrollment for this patient.
- Click Confirm status. The patient’s enrollment status will be updated from “enrolled” to “enrolled with override”.
Unenrolling a patient from the LCS program
By default, all patients in the Lung Cancer Screening module are automatically marked as “enrolled”. However, you may wish to unenroll a patient from the LCS program due to the patient being diagnosed, deceased, ineligible, or asking to opt out.
- Select an entry from the Pending review worklist to open it in the Patient View.
- Under To Do, select Confirm patient’s enrollment status to open the Patient Enrollment dialog.
- Click Enrollment Status and select Unenrolled from the drop-down menu.
- Click Reason for unenrollment and select a reason from the drop-down menu:
- Patient deceased
- Patient diagnosed
- Patient opted-out
- Patient is ineligible
- Other
- (If “Other” selected as reason for unenrollment) Click Please specify reason and enter the reason why you’re unenrolling the patient.

Click Confirm status. The patient’s enrollment status will be updated from “enrolled” to “unenrolled”.
💡 This will also mark the overall entry status as “rejected” and move the entry to the “Unenrolled” worklist.
Confirming the detected Lung-RADS category
Following an exam, the Lung-RADS category is automatically populated from the report content. When confirming, the text used by the AI model to determine the Lung-RADS category will be surfaced at the top, with the entire report available below.
Navigators are responsible for reviewing and confirming the Lung-RADS category.
- Select an entry from the Pending review worklist to open it in the Patient View.

Under To Do, click Confirm detected Lung-RADS category to bring up the Lung-RADS Category Confirmation dialog.
💡 The detected category is displayed as part of the to-do item. - Review the Lung-RADS category, report text used by the AI model to determine the category, and the due date.
- Review the S-modifier present checkbox. This box should be checked if a finding unrelated to lung cancer has been reported for a LDCT exam.
- Do one of the following:
- If the Lung-RADS category is correct, click Confirm.
- If the Lung-RADS category is incorrect, click the Category drop-down, select the correct category, then click Confirm.
- If the Lung-RADS category is correct, click Confirm.
Entry actions
Entry actions are tasks you can perform on an individual patient entry, either from a worklist or within the Patient View:

Registry
The Registry is where you prepare, submit, and track Lung Cancer Screening records to the American College of Radiology’s (ACR) Lung Cancer Screening Registry (LCSR)—helping programs maintain quality, benchmarking, and accreditation readiness.
Our integration with ACR’s National Radiology Data Registry (NRDR) allows navigators to submit screening records directly from Rad AI Continuity with one click.
Records in Registry represent a patient and single instance of an exam (similar to entries in the Worklist area), and can have the following statuses:
- Pending: The record has missing or invalid required/conditional fields. It needs navigator review and data completion before it can be submitted.
- Ready to submit: All required fields pass validation and the record is eligible for submission to the ACR.
- Under ACR Review: The record has been submitted to the ACR and is awaiting a decision/feedback from the registry.
- Submitted: The registry has accepted the record.

Rejected: The registry rejected the record with error details. Fix the flagged fields and resubmit the record.
Here’s the recommended workflow for nurse navigators:
- Select the Registry tab from the main navigation menu at the top.
- All records are shown by default. Do one of the following to filter the list to show only “Pending” records:
- Click the Pending info card at the top of the page.
- In the filter record status filter, click the drop-down arrow and make sure Pending is the only selected status.
- Click the Pending info card at the top of the page.

Click a record in the list to open Record details. Fill out the required fields, then click Save. The record status will be updated, from “Pending” to “Ready to submit”.
💡 Rad AI Continuity automatically populates fields based on available exam and patient information. - Click the Rejected info card at the top of the page (or use the filter) to show rejected records.
- Open each record to review the error(s), correct the data, then click Save. The record status will be updated, from “Rejected” to “Ready to submit”.
- When ready to send, click the Ready to submit info card at the top of the page (or use the filter).

Use the checkboxes on the left to select the records you’d like to send. You can only select records in “Ready to submit” status.
💡 Click the top checkbox to select all “Ready to submit” records. - Click Submit {number} records to Registry, select the receiving Facility, then click Login to Facility to Submit.
- Log in to your ACR account through the authentication portal and submit.
- Use the filter to show records that are Under ACR Review and Submitted and monitor those records.
- Repeat these steps regularly. Use the status filters and top‑of‑page counts to prioritize remaining “Pending” items between submissions.
Analytics
The Lung Cancer Screening analytics dashboard displays data on program composition, performance, and outcomes, including:
- The total number of entries that are pending review
- The number of initial and recurring LDCTs this period
- The number of initial, recurring, and total LDCTs over time
- Resolve rate, broken down by Lung-RADS subcategory
- The number of entries that are currently overdue for follow-up, broken down by 1-30, 30-90, and 90+ days overdue
- Entry status counts over time
- Lung-RADS subcategory distribution over time

How do I change settings in Rad AI Continuity?
Click on your profile icon in the top-right corner, then click Preferences. From here, you can configure keyboard shortcuts, automatic next entry navigation, display options, and more. Learn more.

This may be because:
- You’re not in Rad AI Continuity’s Lung Cancer Screening Module. Learn how to switch modules.

How do eligibility and enrollment differ?
Eligibility indicates whether a patient meets U.S. Preventive Services Task Force (USPSTF) screening guideline criteria (e.g., age, smoking pack-years, symptoms, and smoking status).
Enrollment indicates whether the program is actively tracking the patient for follow-up imaging. They are related but not one‑to‑one: a patient can be eligible but not enrolled (or vice versa) based on program and navigator decisions.
What is an S‑modifier and where is it captured?
An S‑modifier indicates a finding unrelated to lung cancer that appears in an LDCT screening report. S-modifiers can be tracked for patient and provider communications, and for ACR LSCR submissions.
What analytics are available for LCS?
The Lung Cancer Screening analytics dashboard includes data on program composition, performance, and outcomes. Visualizations include initial/recurring LDCT counts, resolve rates, and breakdowns on Lung‑RADS categories, overdue follow-ups, and entry statuses. Learn more.
Why do I not see a "Resolved" worklist like in the Standard module for Rad AI Continuity?
Unlike the Standard module, the Lung Cancer Screening module tracks ongoing patient screening. If a Lung-RADS designation is present in a report, a patient should continue to return for follow-up screening based on ACR guidelines until they are unenrolled from the program (due to cancer diagnosis, patient opt-out, etc.).
As such, a patient that completes a screening exam will have a new entry created with the same or new Lung-RADS designation appearing on their timeline. This will move the patient back to the “Pending Review” worklist to manage the next phase of their screening journey.
If a patient should no longer be tracked, unenroll them from the LCS program. This will move the relevant entry to the “Unenrolled” worklist.
